ELDERLY Oldham residents will have access to improved community care from a new support team.
Pennine Care NHS Foundation Trust and Age UK Oldham have created Promoting Independent People advisers, who will help older people to take control and enjoy a more independent life.
Age UK’s PIP advisers, funded by Pennine Care, will work with community health and social care teams and local GPs. Older people will be referred by doctors if they could be at risk of feeling isolated or anxious about their health issues and need help to regain confidence and independence.
Donna Speat of PIP said: “Plans include strategies to help manage long-term health conditions and re-engage people into the community and supporting them to join groups and activities.”
Advisers will also offer practical support to help keep homes warm and safe, give advice on energy saving measures and work with handymen and shopping services.
For more information, speak to your GP or contact Donna Speat on 0161-622 9277 or pip@ageukoldham.org.uk.
